Job Title: Nurse Practitioner – Clinical & Operations Lead
Location: Office in Brevard County, FL (Patients are seen with In-Home Visits)
We are an in-home medical group seeking a motivated and autonomous Nurse Practitioner to provide transitional care to patients recently discharged from the hospital, with a key focus on reducing hospital readmissions. This role involves travel throughout Brevard County to patient homes and includes a unique opportunity to contribute to and help manage business operations.
In addition to clinical responsibilities, the ideal candidate will have a strong interest in learning and contributing to the backend of our healthcare business, including administrative workflows, referral coordination, and relationship building with external partners. This is a great fit for a nurse practitioner with entrepreneurial spirit or leadership aspirations.
Clinical Responsibilities:
- Conduct in-home patient assessments and develop individualized care plans aimed at reducing readmissions.
- Maintain timely and accurate documentation in the Electronic Health Record (EHR).
- Perform follow-up care via telehealth when clinically appropriate.
- Participate in on-call coverage for select weekends each month.
Operational & Business Management Responsibilities:
- Oversee day-to-day office operations including patient flow, scheduling protocols, and coordination of services.
- Assist in the development and refinement of care protocols and back-office systems to improve efficiency and compliance.
- Serve as the liaison for referral sources (e.g., hospitals, SNFs, case managers) to build and retain strong working relationships.
- Monitor performance metrics and contribute to business development strategies to enhance patient care delivery and growth.

- Schedule: Full-time with rotating on call
- Compensation: Salary and Bonus Based on business performance

Requirements:
- Current APRN license in Florida.
- Autonomous status very highly preferred.
- Must carry your own liability insurance.
- Reliable transportation and willingness to travel within Brevard, Volusia and any additional counties we may add to the business structure over time.
